2. How and when will I be billed for services?
If applicable, you will receive a bill from our office after services have been rendered and your insurance has been billed and paid. It is your responsibility to contact your insurance company if you have not received an “Explanation of Benefits” (EOB) within 30 days of your service. 4. How can I best prepare for my appointment?
Remember that it is important to always tell your doctor information that relates to your health, even if it makes you feel embarrassed or uncomfortable. Be prepared to explain to your doctor what issues and problems you are having. Also, bring an up-to-date “health history” list with you, along with a list of any medications you are taking, including when and how often you take them and their dosages. Bring any other health information, such as test results and medical records.
